Most of this is covered in various theory threads, but you could try:
1) Moving "try through balls" to "never". It sounds like the defence are constantly trying through balls and just not doing it.
2) Increasing the passing length to encourage a further boot upfield or...
3) Decreasing the passing length encouraging a clearance rather than a pass
4) Decreasing mentality to make the defenders more aware of their defensive duties. If in doubt, hoof it
5) Increasing tempo to encourage a quicker decision, or...
6) Decreasing tempo to make the defender not rush and choose a poor pass
