I have a young MC who is 17 years old, has Creativity 20 and Passing 18, but all the other attributes are all under 10 and he has a few 1's in some. But just with those two outstanding attributes - would that be enough to make him a playmaker?

IMHO stylish flairy playmaker doesn´t need high values in teamwork, about 10 is OK, but every playmaker need a high decisions. He operates in small spaces and he needs to decide what to do.

Accelration, Pace are useful, but you can play with slow motion playmaker (Riquelme, Tsartas, Valerón).

A play maker is for creating chances and dicating play for others, id have thought teamwork is crucial for it to work consistently

If he has decent PA then yes in about 4 seasons he'll be a great playmaker for your side.
Cleon: Yes, Teamwork is useful, no doubt about that, but not crucial. If playmaker is good very good individually, he can be a little selfish. The most importatnt thing is, that he recieves most of passes. Then he needs to decide what to do. Create for others, make a dribble or shoot. For example Rivaldo is not a "Teamplayer". But he is so great technically, that for the team is a good thing to play most of passes to him. And there are others. I think even Riquelme's teamwork attribute is about 10, at least in the game.
I have to diagree with Cleon as i use Alonso as my playmaker and his stats are not good in a lot of the stats he mentions.
I play a 4-4-2 with Alonso as DM. This gives him lots of space to dictate the game. Last match v Newcastle, he didn't actually get an assist (rarely does) but he made over 170 successful passes and 35 key.
Also i've found that when i use a playmaker further forward, all the AI needs to do is make sure he doesn't get much of the the ball and my game plan is finished. With the playmaker at DM, he naturaly gets time and space as teams automaticaly back off when you get the ball on FM.
Playmaker in DMC position can easily start most of attacks (I tried that, 90+ passes per game), but playmaker in AMC position is in better position to provide final killer passes. Personally I prefer playmaker in AMC position. And if oppostion concentrates on him, it´s even better, because other players have more space, mainly my other creative player in midfield.
