IMO this happens because somewhere you have config.xml file which points to non existant graphics file.
If there is more than one picture per player, they will show up randomly.

It is not your facepacks fault, it is someones else.

I keep getting this issue, is there any debug commands to wittle it down? Or some easier way to figure out the problem?

Now it's gone, I deleted my cache and switched it off. Seems to cause more problems otherwise.
