I've always tried to create a tactic that uses a Sweeper efficiently, and I believe I am onto something (finally !!).
I've used a variation of this tactic with Dortmund for about 4 seasons (last 2 seasons I won the Bundesliga but nothing else with an impressive squad avarage age of 20 and 21).
And now I started a game with Ajax.
I've started using the first beta version of this tactic after I almost lost my job due to having 2 wins, 2 ties and 3 losses in my first 7 league games.
Anyway enough talk, onto screenshots:
Formation:
Instructions: these are global instructions which I tweak every game, so it's a bit pointless to show them (except the Set pieces settings and the bottom settings).
Team: well I had to buy a few players during the winter break because my Chairman decided it's a wise idea to sell my playmaker, Sneijder. I bought a replacement for him, which was Wagner (from Cruzeiro). I also bought a DMC and it was Apiah (from Fenerbache) and I bought a SW/DC named Chicao (from America (MEX)). The other signings were just youngsters which I plan on using later on.
And now individual settings:
GK: this I chose the preset that comes with FM
SW:

DMC (the left MC):
AMC (the right MC):
AML and AMR:

This was the league standing (keep in mind 3 of the 4 losses were when I was using a diff tactic):
Also my team avg age was 24 (according to the news item that told me that I won the league with an average age of just 24).
Now onto the discussion.
Special instructions that you cannot see directly from the SSs:
I use target men, but not the target man you would think of. My target men are the two wingers, but mainly the left one (Marica). I found that this way I can get a lot more crosses in and I can split the opposing team defences much better.
One thing I like a lot about this tactic is the fact that it's so versatile. For example if I am behind and wish to push for a win or a tie you can sub the SW for a DMC or another FC/ST (based on what you want) and of course take the Farrows and make them Barrows on the full backs (otherwise you'll be getting a lot of goals scored against you). Now if you want to defend a score a lot, you can always sub your ST for a DMC but make sure your AMC gets a Farrow. I found this very usefull in the last 10-15 mins of a game that I really need to preserve a score (and I had a ton of counter attacks that were succesfull).
With the way my team is setup and the players I have at my disposal I found that Stam is almost always getting out paced, so I'll need to get a replacement for him. Probably if you want to try this tactic you will need pacy center backs.

One thing I forgot to mention and I believe it's quiet important.
If you look at my individual instructions, most players have mixed for everything or almost everything.
This is due to the limitations of my players, they aren't the best at for example taking long shots so I don't want to force them to shoot every time but rather let them choose when the best time to shoot is. Same goes for the other selections.
Fantastic to see someone has finally got a sweeper system working! I am a massive fan of the dutch and german leagues and have never been able to implement a successful sweeper system.
I am going to test yours and come back with my ideas.
I like the idea of this systemand I am going to try it myself. I notice that you have targetman supply to run onto ball. Is this just because you have a quick st or a decision to get one? I am not sur who I will do this with yet but I fancy trying it in the prem as it is so different to anything used by any other team.

Actually I have very fast and skilled wingers, and they are the target men (not the striker). Usually I leave it as my AML to be the main target man and then my AMR.
I don't think any team in prem has a sweeper in their squad, but it isn't hard to find a good one
The sweeper for formation you have listed there looks a bit too offensive for my style. Do you find yourself scoring a lot and getting goals scored against you a lot?
Also why do you have your sweeper set to forming a wall on defending free kicks?
Just started my second season. Only changes I made to team was a defender Tamas (my board again decided to sell Heitinga) and a striker Marica (again the board had to sell my star striker Huntellar).
I won 5 games out of 5 in the league, with 12 scored and 1 conceded.

I do score a lot, and don't concede much
BUT, i do have a very good keeper (Ustari). Without him it's a lot more difficult, but still, if you set up your instructions vs. the oppenent well, you will do great.
I had Milan on their knees, letting my whole defenseline (not sweeper) manmark, incl the DMC. The whole attacksquad of milan got a 6.
The set piece instructions were't set at the time of posting. Right now it's set on "default" i believe.
Yeh the main problem I have is the keeper, he is utter garbage. So next season or maybe during the winter break I have to bring a quality one.
To add to my previous posts, I find that I score a lot from set pieces, both free kicks and corners (as I changed all the default settings) and I have a fairly decent free kick taker (17 to free kicks). I probably score at least 1 goal from free kick every 3 games, and at least 1 goal from corners every other game.
