All of them. It makes the "world" more realistic. The more the better I say. Otherwise you see the same teams from the non-selected leagues in the same competitions every year. Also, when you do play teams from other countries they have fewer fake (greyed-out) players, which means you have the opportunity to track more players throughout their career.

LLM is all about simulating the trials and tribulations of a real manager. If you think it's realistic then go ahead and use it.

I'd try a few games on slow. You can react to things quicker and you have more time to analyse the game. If that bores you then pick a faster speed. I use all of them except 'very slow'.
