If it is the first offense ever of that player I give him only a warning for unprofessional behaviour. Additionally I drop him from the squad because of his low fitness. If it wasn't the first offense he will get either a 1-week fine, or, if he isn't improving, a 2-week fine and a direct ticket to the transfer market, sometimes even for free.

I ignore it, but typically sit him for the next match (because his fitness is usually ****e then. )

I'm only concerned with players turning up late or not at all.

Quite right. If a player can somehow still turn up for morning training after a night on the lash, he gets my full respect.
