just looking at your CB's would suggest a lack of pace which I think is vital to be succesfull in the EPL.
Get some pace and set your CB's to defensive, no forward runs or run with ball, tight marking.
Depending on how you play your fullbacks I would set to defensive but leave player instructions all in middle ( mixed ) if you want them to go forward. Its not ideal to have them too attacking because you will get exposed on the flanks, especially in EPL
Do you see any pattern when and how you usually concede? Does it happen more often after crosses, through balls through the middle, long shots, etc.
What kind of instructions your back four have? Are you wingers on Forward Runs Often? What about team mentality / individual mentalities line by lines? Where is your defensive line?
Check after games stats. Any players have many tackles and headers attempted but not too many won? What about interceptions? Your FBs and DM generally should have relatively high higher number of interceptions per game than DCs.
By answering these questions you can try to figure out what's wrong or simply provide your answers here and people can help you out.

Insufficient pace at the back is fatal at all levels, I am finding. I think this aspect has been magnified from earlier versions (not a good thing, IMO).

Agree, except if your backs have less pace than the forwards they have to cover, in which case tight marking gets them pulled out of position and they lose the resulting footrace to the goal.

When having tactical problems, it is important that you watch the match at least on extended highlights, and preferable for the whole match. It's the only way to tell what's really going on.

I've never been a fan of putting DCs on tight marking but I know a lot of people do. I do use it on my FBs though, as most of the replies said mentality, creative freedom and closing down should all be pretty low for the whole back four to start with. If I need a goal I release the FBs more and put the forward runs up.
Even though my DMC has the same mentality and creative freedom as the FBs I do have his closing down higher than the back four, which I've pretty much always had on rarely/just before it changes to mixed.
But you might be doing all of that already!
I figured. But it's amazing how many players post frantic "Why is my team LOSING????!!!" threads and they've never actually watched a match all the way through.
