This happens in virtually every saved game in which I have a team flying high and in position for promotion. There has always been suspicion that opposing clubs 'crack' your tactics after six months or so. I wouldn't say that this is the case, however, because I've been able to use the same tactics successfully for multiple seasons.
But it's inevitable that your club will stop scoring, will start conceding goals, and will slip in form as you get into the final stages of the season. The way to avoid this is by rotating players, altering your training scheme, and paying close attention to what the opposition is doing. Sometimes you can boost morale and get your team playing better by renewing contracts or using media interaction.
You don't have to change your formation necessarily, but some tactical changes can help in terms of team instructions and altering your style of play a bit. The key is to watch the match highlights and determine where the opposition is suddenly getting an advantage.
There is no simple answer, but I can assure you that this tendency to go into a late-season slide is commonplace and is a continual challenge in the game.
