I've noticed that a large number of goals I concede are the ones that come off my keeper and straight to the feet of the oppo for a tap in, this doesn't seem to be the case up the other end, is this just a statistical problem or have all my keepers got the same deficiency that the oppo's keepers haven't?

What's your keeper's handling stat? Have you tried a new keeper?

This one is 18 last one was 19, just seems to happen a lot, maybe my defenders aren't switched on enough to cover?

A lot of the user created tactics on the forums rely heavily on scoring rebounds. This happens a lot and is probably a known issue that could be fixed in the next patch.
