I saw Newcastle come close to it in FM 2007 but that's the closest.

In FM2007 I saw some bizzar things:

AC MIlan between 2011-20014 went as far down as Serie C1 and back upto Serie A

Aberdeen went down to Division III, but obviously couldn't go 'non-league' as this doesn't exist in Scotland.

Man Utd were in League One for a season.

Chelsea spend about 6 seasons in the Championship

Forrest Green were in the EUFA Cup when they won the FA Cup in 2019/2020. they were in the Championship

Liver only went down as far as League 1

A friend of mine played out a very long career with Rushden on FM06, I think it was, during which time both Chelsea and Man Utd were relegated from League 2 (lowest playable league). I believe that they both came back into the league almost immediately though, but never amounted to anything thereafter.
I wouldn't be too surprised to learn that he had some hand in it though, since he has regularly been guilty of using save editors to put clubs in debt, etc. and he has a strong dislike of both teams.

In my own current game, now in 2018, the biggest shock is that Tottenham are in League 1.
