A couple of points that hopefully help.
1. Having your DM close down always doesn't really help against Man Utd (or similar teams). Because of their passing ability, they can easily pass the ball around a single player closing down so it really just uses up your DM's energy.
2. Keep the ball more by reducing the length of passes and slow down the tempo. Having the setting at counter attack just makes your team take advantage of having the ball by getting forward faster and I have never been able to make this work to my advantage.
3. Sitting deep helps reduce the effectiveness of the wingers (Ronaldo, etc), but isn't so good at stopping the long shots from Anderson, Hargreaves, etc so play at a medium setting, but remember to have all your players close down or none of them.
4. You say that the DM doesn't go on forward runs. This is fine, but who is supporting the attack if doesn't go forward? Are there enough players to help hold the ball up long enough or is the opposition defence getting it back quickly and putting you under pressure right away. Think about getting both strikers to hold the ball up more and allow the DM to go on forward runs now and again (middle setting).
5. Try not to make the tactic too complicated by having many individual settings. You can see what is going wrong easier if you set the mentality/creative freedom for the team.
This in itself is not enough to get you clean sheets, because what you also need to do is to watch more games on extended (at least) until you know why they are scoring against you.
It is possible though with alot of work - but that's what makes it satisfying when you get it right.
