1) Warn/ fine players when they get red cards
2) Don't complain about refereeing decisions
and you shouldn't get this.

has anybody noticed any trends in warning/fining players for getting sent off for two yellows as opposed to a straight red?
or do you treat them both the same?

I have warned players for first sending off for two yellows, then fined them; but when someone got a straight red for violent conduct I sent them off straight away.
None of my players have yet complained. When one of my key players got a 2nd yellow I appealed it - the appeal was rejected and 3-4 players started to get concerned about discipline. So I don't bother now.

Not sure, but take a look at your coasches-physio discipline stat. If most of them are low, it can contribute to the problem.
