erm yeah you need to use the wizard thing when creating a new manager and there is an option to add a picture and then its fairly straight forward

Like it has been said above, you can attach a picture to your profiel when adding a new manager, but it is also possible to do it after the manager has already created. Just look how normal facepacks work, and edit the config.xml.
