whats your defensive line setting. If it's too low compared to the high closing down of your dm's (and other midfield players) then space appears between defence and midfield which can be exploited.
TBH I don't really know though, perhaps you should give more details of defensive ine, closing down etc.
It's dd because you are playing a very defensive formation.

Well my back four all have relatively low closing down as I wanted them to keep their shape as best as possible. My defensive line is set at push up but only because I have fast defenders who can deal with balls over the top.
