Have a look in the "patches for dial up " thread where someone has split the patch into smaller parts to download.

AFIAK, you only need the executable and the skin for the patch to work.

So try to get hold of the executable (maybe someone can upload it) for the latest version and modify the existing skin to have 15 slider options. This should work fine.
