They are clearly calculated in a wrong way. Too many things get in the way.

This kind of result happens too much in the game. And generally players are more likely to miss (by kicking weakly to the middle of the net, and somehow the goalkeeper always knows to stay in the middle) than score.

In reality, penalty usually = goal.
In FM, you have a better chance of scoring from a free kick, honestly.

BTW, all of those players have 14+ penalties and 15+ composure and 14+ decisions. I have some very good penalty kickers in my arsenal exactly for these situations. And I know sometimes things are just unlucky... But it shouldn't happen that often.

I'm not pleased with penalties in FM. (In the last couple versions)
