Have you read through threads by Clean, WWFAN, Rashidi and a number of others? If not, go take a look. Print one or two of them out. You will find a lot of helpful things.
Also, without knowing a whole lot more, what is your team tempo? That could be causing problems up top and on your back line if it is too high in games that it should be lower. (try replaying a game with these changes and see what happens.)
Also, is your passing broken up backline - direct, mid - mixed, strikers - short? Short passing b/t CBs is bound to cause problems. The pat it back and forth and the striker steps in, steals, etc.
Hard to tell what the main issues are here without more info, but those are just some easy suggestions.
