In Northern Ireland we only have 3 subs. As I don't want to be left hanging if the goalie gets a foot in the balls, I usually use one spot for a 2nd goalie. We're playing Newry at home, and are leading 1-0 with 25mins to go. I use the 2nd sub to put on a striker coming back from a month out with a twisted ankle. 2mins on the field and he's off again. This time a broken hand, and he's out for two months
The only bloke left is the goalie, which I leave upfield so he will do the least damage. 2mins later (again) - freekick on 30 yards. Goalie hits it, right-winger heads it.... GOAL!
Don't underestimate the power of attacking goalkeepers. An alternative to left-sided sweeper perhaps.
On a bit of a sidenote.. Keepers hit the ball terribly far in this version. Not uncommon they hit it 70 yards to put a striker or winger clean on goal. A bit unrealistic.

Yeah, I've seen that play develop once too many times for my taste. Doesn't help that it's usually against my team.

In Germany, there is a butthead goalkeeper who shoots every penalty his team gets awarded and usually scores too - excellent ratio.
