It might be something you're doing, actually, either in your training regime or in the way that you select your penalty takers.

So to have a guy successfully training for penalties, you need to have him on Shooting and Set-Pieces (for composure and penalty taking, respectively).
You might consider bumping those two items
up a bit for your five-to-seven 'top' penalty takers, if penalties are that important to your side.
Alternately, you might look at how you're selecting your penalty takers. For my usual penalty-taker choice, I pick not the best "penalty taker" full stop, but a player who has the best blend of Penalties, Composure, and Technique.
However, when it comes to a shootout, I tend to put my guys with high Composure 1st, 4th and 5th, figuring those are the highest-pressure times, and I try to 'hide' my guys with middling Composure at 3rd and 2nd, if I have to. It seems to work for me - I've never had 'all five takers miss' or anything like that.
