I put whoever is the best at get a head on the ball (usually highest jumping, but apparently height does matter) on the near post (I'm guessing for you this is Vidic or Rio). Put a strong player with good heading on "keeper." He is here mostly to make it difficult for the GK. Put one striker on Stand at Far Post and the other at "keeper." I put the striker with the best arial ability on keeper. The strikers will poach a few loose balls. Set your best (non-striker) long-shot to Lurk outside of area. If you use a striker he will be man marked, but if he is a midfielder or defender he will usually be left alone. Set two players to back and the rest to forward. If you set players to "run from deep" or "attack far/near post" they will bring defenders too close to the outside lurker.
The real question is how to get the ball to the lurker. There is no corner option Using a corner taker with high creativity and set with fairly high creative freedom helps, but not as much as I would like. I usually set to Near Post Flick or Penalty Area. If I have an advantagea at the near post (pay attention to who is marking at the post and take a look at their attributes) I'll stick with the flick...it can be very effective. My corner takers will still pass to the lurker every now and then. I have a suspicion that the match engine may have been tweaked to be biased against passing to unmarked lurkers. Which is not a bad thing, as you can't defend really defend against it either.
