Ai teams usually go 4-2-4 when chasing the game, so I always go to a flat back 5 late in the game to nullify the four strikers. employ a sweeper if you've got a decent one. I pull my Cm's into the DM position but leave my wingers forward. Click counter attack, defensive mentality and slow the tempo usually helps. Sometimes I set passing to long to ensure that they hoof it away as opposed to playing it out. I've had good success at holding onto leads by doing this. In my current game I cant stop winning 1-0. I think it also helps if you've got defenders with good concentration, composure and determination.

I tend to use a back four and a DM, but the kind of changes I make would apply to most setups I guess. As well as being reasonably solid, they strangely have a tendency to score an odd breakaway goal as a bonus!
Basically, I drop Mentality, Creative Freedom, Tempo and Width right back to around three/four from minimum (you could go more if you like, but this seems to be OK), set passing to halfway or just above and drop the defensive line to about three below centre. Then push timewasting to max. Uncheck Counter Attack, Target Man, Playmaker and Tight Marking too, if they're on.
Sometimes I also tick Offside Trap, depending on what kind of team I'm facing and how good my defensive unit is. If the defense is poor and can't hold it's line, it might be worth dropping the D-Line back a few more clicks instead.
Generally works for me. This is what I'd use against superior opposition and I have a reasonable record there. It's not guaranteed, since it only takes one lucky flick or a quality attacker to pick the lock, but it'll usually hold.
