To answer you question personally I don't set my defenders mentality different to the rest of my team, although I might consider it when the AI is defending as they usually put one striker on a high individual mentality to hit you on the break.

Yes, I currently include the goalkeeper in my team mentality.
I admit the one player I do have trouble with getting good ratings for is my GK. They play well and keep clean sheets, but their ratings stay in the 6-7 region. They usually improve against bigger teams so I put it down to them not having to do much.

It's usually a sign that you are a weaker team if your goalkeeper is getting high ratings. It is indeed because the better your team, the less he has to do.

I use GM all the time now, though I leave my GK on defensive mentality. I originally did it because Rule of One took too much effort. It's quick, simple and easy, and your team all play together.
