It's what constitutes a defensive mistake that I think needs rework. If a defender makes a last ditch goalline clearance, which the commentary even says was amazing work, yet the ball falls in front of a strikers' feet outside the area and he puts it in the back of the net (while the defender is probably still lying on the floor and the goalkeeper is out of his goal) the goal will oftentimes be blamed on the last defender to clear the ball, no matter how brilliant the clearance.
