Most likely done by grabbing a photograph and painting on top of it in separate layers using a tablet. I know one thing for sure though; it wasn't achieved by just applying a few filters.

Can you explain just some steps to achieve this kind of effect in photos?
I'm not so noob as you may though in Photoshop, but off course i'm just working what i've learned until now, so if you don't bother, i would appreciate if you explain just some things to make this effect. 8))
