That will be why. If you play ultra attacking with unrealistic formations you're more likely to get unrealistic outcomes.
In real life teams rarely play ultra attacking, quick football. The best teams generally look to play measured possession football mixed with the odd speedy counter attack.
That said, the number of one-on-one chances being created by certain tactics is a known issue and is being looked at by SI.
In your case though I think your tactic is causing you to create too many rushed chances that look clear-cut, but actually aren't.
Slow down the tempo and look to keep possession more rather than playing ultra attacking.
