Most of the time, yes of course. But there are exceptions to how they are scouted. For example if a guy is mentioned as dangerman and my scouts haven't had a look at him yet, I sometimes call them so they check him out, especially if he's playing in a position I need backup/strengthening.
I will NEVER sign a player without scouting him. As I have two scouts I usually let them both have a thorough look at all targets. Doesn't prevent me from signing hopeless players, but reduces the risk.
Player search is out of bounds for LLM. I don't care what reasoning anybody comes up with. It's out of bounds. Period.

I usually go for 2 report cards, personally, as I've found that it gives me enough detail in a hurry. Otherwise, as long as I have the time then I'll go for at least 1 match report. That way you'll pick up any potential problems or potential good things too.
In answer to your original question, a report card is definitely enough to consider a player scouted - you just might not know the full story.

Does the report card come as a result of scouting? If it doesn't, I don't see how it could be considered as a scouted player.
To answer the question, is it realistic for a team to get a vague report and sign him? If you ran a real team, would you take someone's word for his skills, or would you send one of your employees out to give him a look, and tell you what they thought?
