I don't think it has to do with cities, honestly. It is a fact that most of the major clubs are in the big cities, true. But they are not big clubs because they are in big cities, but because of their success, their players etc.
I am sure if in 5 seasons Norwich won the Champions League, and got Ronaldinho, Ronaldo, Messi and Fabregas in their team, lot of people from around the world would know about it, more than the people who know about smaller clubs from London.

| Yes maybe, but they need to be consistently succesful, for many years. "Worldwide" status can;t just be thrown around lightly. It's for the cream of the crop. Maybe you need to win a World competition. World Club Cup or Super Cup or whatever it's called.

I've analysed reputation in an experiment, and from what I can see:
The words 'continental, worldwide' are just a reputation of reputation, which I believe goes up to 10000. So in theory, your reputation could drop by just one point (0.01%) and you could slip from worldwide to continental.
Worldwide is actually a very hard reputation to maintain, and usually requires consistent European success. One slip and you'll lose enough reputation to drop away.
Are you playing the Italian leagues on full details? I have a suspicion that teams in leagues that are not playing in this way maintain their previous reputation no matter what. I may be wrong there.
But I found that Premiership clubs (in my experiment I play only the English leagues) only hit worldwide of the back of a spectacular season, e.g Premiership win and Champions League semi+.
Basially, Worldwide requires a massive reputation, which is incredibly difficult to maintain. Don't worry about it - if you're winning titles you'll still be streets ahead of the rest of the league.

| There's no such thing as having a permanent reputation in an active league. To maintain a worldwide status in my experience you have to win at least two major competitions and advance far in the Champions League. Look at Chelsea in my game:-
Chelsea, Reputation 2nd August 2007 9200

Reputation 4th August 2008: 9177Worldwide(23 point decrease)
So despite doing the double they actually lost reputation points because of their performance in the Champions league.
Look at Arsenal:-
Arsenal, Reputation 2nd August 2007 8800

Reputation:8885Continental(85 point increase)
Winning the Champions League only gave them an 85 point boost as when it comes to reputation equivalent performance in your home league relative to previous seasons has no effect on reputation.
It seems that rep works similarly to FM 07 so to keep your reputation at the higher levels that the big clubs start out at you have to pretty much do the Champions League/Premiership double year in year out. You didn't post how you did in the season prior to your reputation dropping but I would hazard a guess that you failed to win either the Premiership or the Champions League. If you can be bothered to post your full history of competition performance from when you took over as Newcastle boss it will shed some light on what happened (Premiership, European Competitions and FA Cup will do).
Also the reputation boost you get from winning competitions is calculated relative to your current reputation which basically simulates comparing current performance to previous performance. In other words if you went from finishing outside the top 7 for four seasons and then won the Premiership you get a much bigger boost in rep than if you had won the Premiership and been in the title hunt in previous seasons.
As an example with Leeds I won the premiership in the previous 2 seasons, both times getting a reputation boost of 400+ points as a result. Reputation on 24th April 2013 is 8192. Win league again and the FA Cup, rep increases to 8452. So I get a lower boost after 3rd title due to my higher rep prior to winning it. Personally I think this system is a bit screwed up as at the moment despite winning 3 consecutive Premiership titles Arsenal and Man Utd still have higher reputations than me.
It seems that European success is the be all and end all of increasing reputation to the higher levels when in the English Premiership. Quote:

| I never play with any other leagues active except the country I am in, usually England. What I have noticed about this is that clubs in non active leagues, their reputation appears to be pretty stagnant. Whether or not setting leagues to 'view only' or 'playable' might also affect this I don't know.

On 07 the reputation ranges were like this:-
Obscure 0 to 1299
Local 1300 to 2899
Regional 2900 to 4499
National 4500 to 7499
Continental 7500 to 8999
Worldwide 9000 to 10000
I don't think they've changed for 08.

The 9000+ reputation mark makes sense, and in my experience the only sides that hit that level are teams that, in the last two seasons, has won the Premiership at least once and reached at least one Champions League final.
Sometimes even that isn't enough depending on performances in other competitions.

Consider the example of Real Madrid.
A few years ago the so-called Galacticos managed not to win anything for about 3 years and yet the club reputation did not really suffer.
They have 50 years of European Cup tradition, countless domestic titles, have the king of Spain as their patron, and famously did not sign Ronaldinho because he was too ugly.
Their reputation should remain as "Worldwide" for many years even without success in Europe, due to their history, star players, the standard of the Spanish league, the rivalry with Barca, the attractiveness of Madrid as a city, and Spain as a country.
To go back to the original post, Newcastle has few of those attributes (no offence  ). It will therefore take much longer for Newcastle as a football club to enjoy the same reputation as Ream Madrid, AC Milan, Manchester United, etc.
Maybe if you played for a few more years it will become easier to maintain a worldwide rep.
