I don't know of any tutorials however, I do have some suggestions of how I'd go about it in Photoshop. First I'd find some image I could use for a reference of the basic lines of an open book and use that as a template or tracing layer at the bottom of the layers palette. I used Google Images and came up with this example which has good lines, I think.

Next would be to get out the old pen tool and start by creating the cover. Adding color and texture to taste. Adding a 3D effect to the cover could be achieved by using Layer Styles or a better job would be to use the ideas found in steps 3 and 4 this tutorial...

Creating the pages could be done by using the pen tool to trace a path and stroking it. The depth on the top pages can be done by using the dodge and burn tools. The page curls can be accomplished 'by hand' by using this excellent tutorial...
