It could be PCS, a fairly "nothingy" complaint with no real risks other than perhaps the pain coming on while, well, doing anything where you can't suddenly think "Crap, feels like a needle's peircing my heart and lungs"...
I've had it myself on and off... Some people find that a big breath "pops" the PCS and makes it go away, never been brave enough to try myself.

muscle strain for crying out loud
every time you breath your ribcage expands causing the muscle to exapand also.
i got sever cramp in my upper back after doing my back/bicep workout a few works ago. just under the arm. couldn't take deep breaths without pain so had to stretch it out
It's not neccessarily that PP. There's lots of potential reasons... fortunately heart attack isn't one of them (or it wouldn't be linked so closely to his breathing).
