there is no possibility its a (albeit sick) coincidence?

its not likely but its possible

Valid point.

In answer to the OP question though - Straight to HR Director, as high as you can go, or the Chief Exec. If it's meant maliciously then you need to tackle it as it is meant - a heartless attack at a person who is vulnerable.
