I like ones that completely change the original song into something altogether more interesting.

To that end I would go with Life of Agony's version of 'Don't You Forget About Me' (a brooding version that is almost sinister to the extent that it's a relief when they eventually unveil that big synth hook) and Type O Negative's cover of 'Paranoid' (which brilliantly ignores the standard Paranoid riff altogether, and when a riff finally does kick in its from an entirely different Sabbath song).
