Sounds like you're using Chelsea, so it's not the players. I'd take this back to the Tactics forum and resubmit. But don't frame it as a formation problem, since it is probably a problem with your tactical instructions (specifically, for the strikers) rather than your formation. This is a 'Why can't my strikers score and dominate?' problem, which lots more people can help with, rather than a 'Does this formation work?' problem.
I'd recommend asking in one of the all-purpose threads too, like rashidi's, instead of starting your own. The catch-all threads get much more traffic and the others tend to die quickly. Tell them the problem (can't score, or can't get good ratings?) and give them plenty of detailed info about your tactical settings to work with. They can't offer good advice if you don't tell them specifically how you've done your settings. (Ideally you can link to a screen capture of the settings to imageshack and let people see them for themselves.) And since the problem may be poor service, or too much space between your midfield and strikers (preventing them from linking properly) you should probably let them know about your midfield settings as well. Also let them know it's Chelsea, so they don't say it's your players.
Knowing nothing about your settings, the first thing I'd suggest is upping the creativity for your strikers to 15 or so. Good upfront players tend to do well when you give them freedom to find space to move into.

I am using two sides trying the 3412 wingbacks formation AS ROMA and Chelsea. I am playing:

So- for both sides i'm aiming my crosses at Drogba/Elemander's post- sometimes Owen is on the post as i've had some success with him scoring like that. Settings:

Passing: 4/20 short (but have tried alternates and have different settings for individuals, such as my flat MC on mixed.)
Mentality: 10-14/20
Creative Freedom: 11-14/20
Width: 11-15/20
Defensive Line: 17-19/20
Pressing: 16-20/20
Individual:
WBR (Mancini/Lahm)
Mentality: 18-20/20
Creative freedom: 16
Arrows to AMR (Lahm + Chelsea)
Arrows to FR (Mancini + Roma)
WBL
Similar- both sides wingbacks have become very agressive in a bid to combat these problems with scoring.
I have one Defensively minded MC with an Arrow back to DMC (De Rossi/Essien) one flat midfielder (lampard/Perrotta) who has become increasingly attacking- but his settings are:
Forward runs: mixed
mentality: 11/20
creative freedom 11/20
long shots often
run with ball mixed
and then an AMC of Totti/Riquelme (or Diego/Joe Cole for Chelsea) who has:
18/20 attacking
100% creative freedom
short passing
long shots
The only possible problems i can conceive are:
- The Long Shots midfielders Issue which will hopefully be patched.
- The Creative freedom of the strikers/central flat midfielders
- The crossing to one post
- A generic Problem with the AMC. A friend plays the same formation with a flat 3 midfield and has excellent success in 08.
My problems are constant 0-0 draws or small scorelines and poor striker ratings.
If my side is in great form at HT they are likely to be... 6: 45% 7: 45% 8: 10%
In average or poor team form they will almsot certainly (80+ %) of the time be on a 6 at the end of the first half. Aguero, Drogba, Owen especially although i had some success with him initially with this formation but then his goals dried up...
They don't score and basically nothing happens.
