This has happened a couple times to me now, when I've discovered at some point in the second half (maybe the 65 minute mark) that the other team is playing shorthanded without having someone sent off. It typically happens when they've made two subs, and then a player get hurts, they chose not to replace that player, despite having 1 sub remaining.
I'm not sure tihs is because they are holding their sub in reserve in case of a goalie injury, or because they have no one who can fill the position of the injured player. Neither makes much sense. I don't know of many teams that would play shorthanded and save a sub. And in the 2nd case, the AI should be smart enough to play a different formation if they don't have the proper player to play their current one (if they lose a winger and only have DCs on the bench, they should move things around).
Anyone else noticed this problem? I just upset Arsenal at Emirates Stadium, with my winning goal scored 12 minutes after they refused to sub an injured player.
Another annoying thing about subs is once you've clicked confirm, you can't change your mind.
So occasionally, for example, you'll bring on a player to protect a lead, click confirm, wait a while and then the opponents will equalise. But you're forced into making a change that doesn't make any tactical sense.
In reality, a sub would be ready to come in, the manager would see the goal conceded and have a rethink.
I haven't seen it, myself, but I did see somebody else post it in GD a week or so ago.
You might take a quick look in the FM'08 Bugs Forum; if you can't find it on the first two or three pages, post it yourself. If you do find it, add your observation to the discussion.
A .pkm file of the match in question will be useful to SI, if they haven't seen it themselves yet.
