I think you'd act, speak and be culturally Italian, but the fact remains that you're not Italian. I just find it hard to envisage a situation where someone would rather play for a random country that they've lived in for a while, than their true nationality.

tbh neither can i, i would only play for ireland, but on the otehr hand i dont see why someone cant be 'adopted' by another nation as you say they would speak the other language, ad be culturally italian, and may very well feel more at home in the other nation, so why should they not be allowed play for the second nation?
I'd play Almunia if I was England manager, just for a friendly or something.
On FM I would often give players who became eligible to play for England a game. This also included Fulham's useless Argentinian striker from a few years ago. Most surprisingly, he scored over 20 goals for me in 15 appearances despite his poor stats. I was addicted to using him.
I know a guy who was born in Trinidad, but moved here when he was two, to him the random country is Trinidad as he doesn't remember it at all, and he's English with a British passport, even though he's only lived here "for a while" that's his 'true' nationality.
