My defenders just hoof the ball upfield after the finally take it away from the opposition!
My midfielders seem to dwell on the ball to much, which results in them just losing it to AI time and time again!
What am I doing wrong?!?!?

Tempo, too much creative freedom??

I'm managing lower league and found that reducing creative freedom helped as pretty much do as I ask.

The Mentality of your defenders maybe too low, which is why they are just hoofing the ball away:

Try DC's with Mentality - 4
FB's with Mentality - 6 - 7

Regarding the midfielders dwelling maybe the TTB is set to often and the players don't have right attributes to find the right pass i.e.Decisions thus losing the ball.

Try setting the TTB to mixed or even rarely if you find they still lose the ball.
