Default Brush size not always shown

my cursor does not always represent the size of my selected brush, even though i have the option on in the settings dialog. this is intermitant and v. annoying. thanks for any advice any one can give

Do you have the hardness set to a value lower than 100%?

Were I confronted with such an issue, I believe I might trash the Photoshop preferences and relaunch PS.

The size of brush will not show in PS if you have Caps Lock ON. Check that out.

That is to toggle between standard and precise cursors. When the Caps Lock is on brush size is showen like cross cursor, to go back to standard you need to press Caps Lock again.
Check the Photoshop Help file : Search Caps Lock and you will find more info for another tools also.
