Two shots both with the same lighting conditions . Then the eyes and mouth are dropped in - with heavy masking. Color adjustments would also be made so that the two images "fit" together.

You could also paste the two images into the same canvas and use a clone brush with the opacity turned down a bit and draw right over the dog's features. (you'd probably have to blank out the dogs eyes first with a paint brush to make it blend well.)

I would thing there was a deformation map involved with the dogs head as well.
